Introduction
These terms and conditions ("Terms") govern the provision of hospice care services ("Services") by Avra Hospice, Inc. ("AVRA"). By using our Services, you agree to be bound by these Terms. We urge you to read these Terms carefully and thoroughly before availing our Services to ensure you understand how we work and what is expected of you.
Services
AVRA is dedicated to providing compassionate and comprehensive hospice care services to patients diagnosed with terminal illnesses. Our Services aim to support and comfort patients in their homes or at our dedicated hospice facilities. We offer a range of services, including, but not limited to, Medical Care Management, Case Management, Pain and Symptom Management, Nursing Services, Medical Equipment & Supplies, Social Work Services, Continuous Care, Respite Care, Support Counseling, and Emotional and Spiritual Counseling.
Eligibility
To be deemed eligible for our Services, the patient must have a terminal illness with a prognosis of fewer than 6 months as certified by a licensed physician. The patient must willingly choose not to pursue curative treatment for their illness. They should be under the care of a physician who will oversee the hospice plan of care. Furthermore, the patient must reside within the service area of AVRA.
Financial Responsibility
AVRA will assist patients in verifying their insurance eligibility and will file claims on patients' behalf. However, patients are responsible for co-pays, deductibles, and any services not covered by their insurance plan. AVRA is committed to providing care regardless of a patient's ability to pay. We offer financial assistance to patients who meet specific eligibility requirements.
Discharge
Patients may be discharged from AVRA's hospice care under certain circumstances, including improvement in their condition thereby negating the terminal illness certification, relocation outside AVRA's service area, voluntary withdrawal from the hospice program, or admission to a nursing home or other facility providing similar care.
Consent for Treatment
Patients or their legal representatives must provide explicit consent to all hospice services recommended by the hospice team. Patients reserve the right to refuse treatments or medications at any stage of their care.
Confidentiality
AVRA adheres to strict confidentiality guidelines. Patient health information is highly confidential and will not be disclosed without explicit patient consent, except as required by law or in situations necessary for the provision of care.
Grievances
Patients have the right to file a grievance regarding any aspect of their care. AVRA assures patients that any grievance lodged will be thoroughly and impartially investigated, and appropriate action will be taken.
Amendments
AVRA reserves the right to amend these Terms at any time. Any changes to these Terms will be posted on our website. Continued use of our Services following any amendments will constitute acceptance of those changes.
